Release of Liability: We, as parents or guardians of the above named individual, hereby grant permission for him to participate in Greenwood Boys Basketball - Fundamental Camp / Shooting Camp and acknowledge the fact that he is physically able to participate in camp activities. We hereby release the Greenwood Boys Basketball Camp and its employees, Greenwood Independent School District, its Board of Trustees, Administration, and employees from all claims from injuries or illnesses which may be sustained by the above named individual and authorize the director or his designee to select hospital facilities and/or physician of his choice and authorize treatment of the above named individual on an emergency basis in the event such treatment becomes necessary while taking part in Greenwood Boys Basketball - Fundamental Camp / Shooting Camp.
Media / Photo / Video Release: We, as parents or guardians of the above named individual hereby authorize Greenwood Boys Basketball - Fundamental Camp / Shooting Camp to photograph, take motion pictures, take video footage, and/or make electronic sound recordings herein referred to as photographic or electronic reproductions. We authorize the use of any such photographic or electronic reproductions of the above named individual for any purpose, including, but not limited to educational and other public media as may be deemed appropriate by Greenwood Boys Basketball Camp. We understand that the above named individual may be identifiable from such photographic or electronic reproduction.
